-Proof of Workers Comp Insurance or Workers Comp Waiver (see below for more information)
-Proof of your non-profit status

    Vendors are responsible to provide their own tent or canopy, weights, tables and chairs. All vendors are responsible for supplying their own electricity. The city will provide the space ONLY.

    Vendor Agreement Acknowledgement - By checking this box you are stating that (i) you are an authorized representative of the organization on this application and (ii) you understand, and agree to the terms and conditions set forth in the Vendor Agreement for each event to which you are applying.

    By checking this box you are stating you understand that if payment is not made two weeks prior to event date your vendor space will be forfeit.

    Insurance Requirements

    Vendors must provide the city with proof of Worker's Compensation coverage in order to participate.
